IF YOUR CHILD EXHIBITS SYMPTOMS ASSOCIATED WITH COVID-19:
PCSD follows the CDC guidance for return to school after students or staff members exhibit symptoms associated with COVID-19.
If your child is experiencing COVID-19 symptoms, please keep them home and follow the CDC Guidelines. They should return to school when they meet the following criteria:
New York State Public Health Law requires that every child entering 6th Grade must have proof of receiving the Tetanus, Diphtheria and Pertussis (Tdap) Booster. Students entering 7th Grade must also have proof of receiving the Meningococcal vaccine. Both of these vaccines must be given after age 10. Please contact your healthcare provider to make sure your child has what they need to attend school in the fall.
New York State Education Law mandates that all students in 7th Grade, and all students new to the district, have a current health appraisal on file in the health office. These appraisals must have been given within the last twelve months. Please ensure that we have a copy of your child's latest health appraisal by requesting that your pediatrician send it to us via fax (249-6420) or have your student drop it off in our office.
New York State Education Law mandates that all 7th Graders have the following screenings completed within 12 months of starting school.
Even though your child may have had a recent physical, many pediatricians are not performing all of these mandatory screenings, especially the near vision screening. Therefore, starting in mid-January of each school year, our office conducts the above three screening tests as necessary for each student, during their P.E. class. Parents will be notified with the results of these screenings. If you would like to opt out of this screening, please let us know by sending us an email. Any requests to opt of the screenings must be in writing and received before mid-January.

The New York State Education Department, Office of the Professions, division of medical licensing does not recommend and strongly discourages medical providers from treating family members. Accordingly, because the district follows a "best practice" standard of care in our health offices, please be advised that the district will not accept medical clearances, requests, forms, documents, certificates, or any other medical notice, etc. from a parent who is acting as their child's health care provider.
All medical orders, certificates, or other required statements or forms must be written by the child's private primary care physician, or health care provider. A parent who is also a health care provider must complete all required parent documentation/ permission to supplement the treating physician or other health care provider's information, the same as any other parent. Your failure to cooperate may delay the processing of your child's information resulting in delays in your child's ability to participate in various activities.
If your child requires the use of emergency medication such as an Epi pen, inhaler, seizure medication, or has a medical condition that requires special attention such as diabetes, please let our office know and complete one of our Emergency Care Plan forms.
