Harassment/Discrimination Reporting Form

Harassment, Bullying and/or Discrimination

Please use the Discrimination/Harassment Reporting Form to report DASA incidents to the District.

The Penfield Central School District is committed to eliminating harassment, bullying and/or discrimination from our school community.  We believe that a student’s ability to learn and to meet high academic standards, and a school’s ability to educate its students is compromised by incidents of harassment, including taunting, hazing, intimidation, and bullying, and/or discrimination.  Employees, students or community members who have witnessed harassment, bullying and/or discrimination, or have reliable information that a student has been subject to harassment, bullying and/or discrimination, are encouraged to report the incident to a school administrator immediately.

The Dignity for All Students Act, effective July 1, 2012, prohibits incidents of harassment, bullying and/or discrimination, whether a single incident or a series of related incidents, against any student by employees or students on school property or at a school function. Any such incident where a student is subjected to harassment, bullying,  and/or discrimination that creates a hostile environment of such a severe or pervasive nature that it unreasonably interferes with a student’s educational performance, opportunities, mental or emotional well-being, or causes a student to fear for his or her physical safety, are prohibited. Such conduct shall include, but is not limited to, threats, intimidation or abuse based on a person’s actual or perceived race, color, weight, national origin, ethnic group, religion, religious practices, disability, sexual orientation, gender or sex.  In addition, other acts of harassment, bullying, and/or discrimination, which can reasonably be expected to materially and substantially disrupt the education process, may be subject to discipline or other corrective action.

Incidents of harassment, bullying and/or discrimination may be anonymously reported by completing the online form linked below or by printing out the form and mailing it to Dr. Tasha Potter, Assistant Superintendent for Equity & Access at Penfield Central Schools, P.O. Box 900, Penfield, NY 14526.
