• PART I - Needs to be filled in by applicant and a parent/guardian signature IS REQUIRED.
• PART II - Evidence of Age is completed by issuing official only (If applicant is not a PHS student, ID is required).
• PART III - If the applicant is a PHS student the School Nurse can complete the certificate of Physical Fitness if the students health records are up to date. If a non-PHS student this section should be filled out by the applicants physician.  
• PART IV - Is not necessary to have filled out.
• PART V - Is not necessary to have filled out.
• PART VI - Employment Certification is completed by the issuing official only.

When highlighted items have been completed, please return the papers to the Counseling Office to receive a permit.
