Attention Parents: Please fill out and sign the Student Records Release Authorization form giving PHS permission to send your student's records, transcripts, and other supporting documents to any colleges, scholarships and organizations that they request. Students must return the signed form to the counseling office. 

GREEN SHEET bring into the counseling office to have your transcript and other information released to colleges. Hard copies are available to pick up in the counseling office.

YELLOW SHEET bring into the counseling office to provide information you need for Scholarships. Hard copies are available to pick up in the counseling office.
